Beechworth Health Service is proud to partner with local health services to improve experiences for patients who are transferred from one health service to another, through the Transfer of Older Persons (TOP) project. The project includes researchers from Albury Wodonga Health, Beechworth Health Service, Tallangatta Health Service, Corryong Health, Culcairn Hospital, @opanaus and John Richards Centre for Rural Ageing Research (https://www.latrobe.edu.au/jrc) at La Trobe University.
Since December 2020, older patients and involved healthcare staff have been interviewed about their transfer experiences. We aim to better understand what works well and how to improve what doesn’t. Later this year, we will trial some possible improvements.
The TOP Project is jointly funded by Albury Wodonga Health and La Trobe University, and is being led by Dr Clare Wilding, Research Fellow in Ageing and Subacute Care.
